How do I request a Mathematica personal-use license?

Mathematica personal-use licenses are a benefit of Premier Service for non-student licenses. They are designed to give users who have Mathematica on a work computer the convenience of using it on their home computer as well.
Personal-use licenses are valid for the Premier Service subscription period only. When you renew your Premier Service, your personal-use license will be extended for another year.
A personal-use license through Premier Service is available on your choice of a macOS, Windows or Linux platform, regardless of your primary platform—for example, you could have Mathematica for Windows at work and get a personal-use license for macOS.
